Sorry, we are unable to supply medication.If the prescription contains the name of a medication listed as a restricted medication then we cannot supply the prescription with the item on it. For multiple item prescriptions please multiply the fee (£6.65) by the number of items on the prescription. Enter the total cost on the reverse of the prescription (part 2). Then sign and date in part 3 on the reverse of the prescription where indicated. Please note: The NHS charge some medicines at 2 or more fees per item and some are always free. If you are unsure, please click to check the list of medicines that do not attract the normal charge. Enter the total cost on the reverse of the prescription (part 2). Then sign and date in part 3 on the reverse of the prescription where indicated.Step 5: NHS Prescription Charge Exemption RequirementsThe patient is under 16 years of age No further proof required if age or date of birth is printed on the front of the prescription. If your date of birth is not present on the prescription then we require one of the following: photocopy of passport, birth certificate, student id card (with date-of-birth displayed) or NHS medical card. In part one on the reverse of the prescription please tick option A. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe patient is 16, 17 or 18 and in full-time education Please provide a headed letter from the school or college, or documents showing payment of child benefit or family tax credit. In part one on the reverse of the prescription please tick option B. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient aged 60 years of age or over No further proof required if age or date of birth is printed on the front of the prescription. If your date of birth is not displayed on the front of the prescription please provide a photocopy of passport, birth certificate or NHS medical card. In part one on the reverse of the prescription please tick option C. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who has a valid maternity exemption certificate We require a maternity exemption certificate or an NHS maternity exemption card. Please provide a photocopy of your NHS maternity exemption card or a completed FP92 or FP96 form. In part one on the reverse of the prescription please tick option D. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who has a valid medical exemption certificate Please provide a photocopy of your NHS Prescription Charge Exemption card or a completed FP92 or FP96 form. In part one on the reverse of the prescription please tick option E. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who has a valid prescription pre-payment certificate Please provide a photocopy of your NHS prescription prepayment card or a completed FP95 form. In part one on the reverse of the prescription please tick option F. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who has a valid war pension exemption certificate Please provide a photocopy of your War Pensions Agency Certificate. In part one on the reverse of the prescription please tick option G. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who is named on a current HC2 charges certificate Please provide a photocopy of your HC2 Charges Certificate or a photocopy of your entitlement letter. In part one on the reverse of the prescription please tick option L. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who was prescribed free-of-charge contraceptives There is no charge for this prescription. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who gets Income Support (IS) Please provide a photocopy of your Income Support Order Book, Giro letter or tear-off portion. In part one on the reverse of the prescription please tick option H. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who gets income based Jobseeker's Allowance (JSA (IB)) No evidence is available - your entitlement will be checked later by the Department of Health. In part one on the reverse of the prescription please tick option K. Below the tick boxes in part one on the reverse of the prescription you are also required to print the name of the person (either you or your partner) who receives income support, your date of birth and your National Insurance number. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who is entitled to, or named on, a valid NHS Tax Credit Exemption Certificate Please provide a photocopy of your Working Families' or Disabled Person's Tax Credit Exemption Certificate or a photocopy of an entitlement letter. In part one on the reverse of the prescription please tick option M. Below the tick boxes in part one on the reverse of the prescription you are also required to print the name of the person (either you or your partner) who receives income support, your date of birth and your National Insurance number. In part three on the reverse of the prescription please sign and date the prescription either as the patient or as the patient’s representative.Step 5: NHS Prescription Charge Exemption RequirementsThe prescription is for a patient who has a partner who gets Pension Credit guarantee credit (PGCG) Please provide a photocopy of your order book or an entitlement letter (if payments are made into your bank or building society) from your Jobcentre Plus office. In part one on the reverse of the prescription please tick option S. Below the tick boxes in part one on the reverse of the prescription you are also required to print the name of the person (either you or your partner) who receives income support, your date of birth and your National Insurance number. This Patient Registration form requests all the information we need to process your prescription.
